Is the flywheel replaced with the clutch?
Only if inspection shows it needs it. Its condition is assessed once the gearbox is out, and you are told before extra parts are added.
Can the fault be hydraulic and not the clutch itself?
Yes. A leaking cylinder or air in the system can cause poor disengagement. This is checked before the clutch is condemned.
